    I’m using http://dreamhost.com, and it provides all the technology you mention. It does have occasional outages (maybe once a month) but they’re usually short and they are very responsive when you chat with them through the website.

    One thing that’s a little bit annoying is that the mysql server is on a different computer and they don’t provide persistent database connections – so every web request is slowed down a little bit by initiating a database connection.

    To avoid this, you might try http://anhosting.com – I haven’t used them, but I’ve heard they’re pretty good.
